  1. Caseous necrosis

Caseous necrosis is a form of cell death in which the tissue maintains a cheese like appearance. ref Robbins and Cotran Pathologic Basis of Disease, 8th Ed. 2010. Pg. 16 ref The dead tissue appears as a soft and white proteinaceous dead cell mass . Causes Frequently, caseous necrosis is encountered in the foci of tuberculous infections. ref Robbins and Cotran Pathologic Basis of Disease, 8th Ed. 2010. Pg. 16 ref It can also be caused by syphilis and certain fungi. A similar appearance can be associated with histoplasmosis , cryptococcosis , and coccidioidomycosis . ref name urlPulmonary Pathology cite web url http library.med.utah.edu WebPath LUNGHTML LUNG031.html title Pulmonary Pathology format work accessdate 2008 11 21 ref Appearance In caseous necrosis no histological architecture is preserved. On microscopic examination with H&E staining, it is characterized by acellular pink areas of necrosis surrounded by a Granuloma granulomatous inflammatory process. When the hilar lymph node for instance is infected with tuberculosis and leads to caseous necrosis, its gross appearance can be a cheesy tan to white, which is why this type of necrosis is often depicted as a combination of both coagulative and liquefactive necrosis. However, in the lung, extensive caseous necrosis with confluent cheesy tan granulomas is typical. The tissue destruction is so extensive that there are areas of cavitation also known as cystic spaces . See Ghon s complex . 1. Coagulative necrosis

Coagulative necrosis is a type of accidental cell death typically caused by ischemia or infarction . It is characterised by the ghostly appearance of cells under light microscopy in the affected area of tissue. In coagulative necrosis the architecture of dead tissue is preserved for at least a couple days. ref Robbins and Cotran Pathologic Basis of Disease, 8th Ed. 2010. Pg. 15 ref It is believed that the injury denatures structural proteins as well as lysosomal enzymes thus blocking the proteolysis of the damaged cells. The lack of lysosomal enzymes allows it to maintain a coagulated morphology for some time. Like most types of necrosis if enough viable cells are present around the affected area Regeneration biology regeneration can occur. Causes Coagulative necrosis is most commonly caused by hypoxia medical hypoxic conditions that do not involve severe Physical trauma trauma , toxins or an acute medical acute or chronic medicine chronic immune response . The lack of oxygen causes cell death in a localised area which is perfused by blood vessels failing to deliver primarily oxygen, but also other important nutrients. It is important to note that while ischemia in most tissues of the body will cause coagulative necrosis, in the central nervous system ischemia causes liquefactive necrosis as there is very little structural framework in neural tissue. Pathology Macroscopic The macroscopic appearance of an area of coagulative necrosis is a pale segment of tissue contrasting against surrounding well vascularised tissue and is dry on cut surface. The tissue may later turn red due to inflammatory response. 1. Acral necrosis

Refimprove date June 2007 Image Acral gangrene due to plague.jpg thumb right 250 px A hand with acral gangrene due to bubonic plague plague . Acral necrosis is a symptom common in bubonic plague . The striking black discoloration of skin and tissue, primarily on the extremities acral , is commonly thought to have given rise to the name Black Death , associated both with the disease and the pandemic which occurred in the 14th century. The term in fact came from the figural sense of black , that is glum, lugubrious or dreadful. Clotting and bleeding beneath the skin causes an area of hemorrhage, the presence of red blood cells lying outside of capillaries, into the skin and subcutaneous tissue. In isolation, this is called an ecchymosis or bruise and may be the result of injury or illness. However, acral necrosis occurs when blood supply is disrupted for prolonged periods, blackening and damaging the affected area and surrounding tissue. With appropriate medical treatment, areas with Acral necrosis may be successfully restored to function and lead a normal life. Untreated cases can lead to death. 1. Liquefactive necrosis

Liquefactive necrosis or colliquative necrosis is a type of necrosis which results in a transformation of the tissue into a liquid viscous mass. ref Robbins and Cotran Pathologic Basis of Disease, 8th Ed. 2010. Pg. 15 ref Often it is associated with focal bacterial or fungal infections. In liquefactive necrosis, the affected cell is completely digested by Hydrolysis hydrolytic enzymes, resulting in a soft, circumscribed lesion consisting of pus and the fluid remains of necrotic tissue. Dead leukocytes will remain as a creamy yellow pus. ref Robbins and Cotran Pathologic Basis of Disease, 8th Ed. 2010. Pg. 15 ref After the removal of cell debris by white blood cells , a fluid filled space is left. It is generally associated with abscess formation and is commonly found in the central nervous system . In the brain For unclear reasons, Hypoxia medical hypoxic death of cells within the central nervous system also results in liquefactive necrosis. ref Robbins and Cotran Pathologic Basis of Disease, 8th Ed. 2010. Pg. 15 ref This is a process in which lysosome s turn tissues into soup as a result of lysosomal release of digestive enzymes. Loss of tissue architecture means that the tissue is essentially liquefied. This process is not associated with bacteria action or infection. The affected area is soft with liquefied centre containing necrotic debris. Later, a cyst wall is formed. Microscopically, the cystic space contains necrotic cell debris and macrophage s filled with phagocytosed material. The cyst wall is formed by proliferating capillaries , inflammatory cells, and gliosis proliferating glial cells in the case of brain and proliferating fibroblast s in the case of abscess cavity.
