Professor Ivan Maurice Roitt born 30 September 1927 was educated at King Edward s School, Birmingham and Balliol College , Oxford University . In 1956, together with Deborah Doniach and Peter Campbell , he made the classic discovery of thyroglobulin autoantibodies in Hashimoto s thyroiditis which helped to open the whole concept of a relationship between Autoimmune diseases autoimmunity and human disease. The work was extended to an intensive study of autoimmune phenomena in pernicious anemia and primary bilary cirrhosis . In 1983 he was elected a Fellow of the Royal Society , and has been elected to Honorary Membership of the Royal College of Physicians and appointed Honorary Fellow of The Royal Society of Medicine . Myxedema psychosis , more colloquially known as myxedema madness , is a relatively uncommon consequence of hypothyroidism , such as in Hashimoto s thyroiditis or in patients who have had the thyroid surgically removed and are not taking thyroxine. A chronically under active thyroid can lead to slowly progressive dementia , delirium , and in extreme cases to hallucination s, coma , or psychosis , particularly in the elderly. It was first recognized by Dr Richard Asher of London in 1949. Treatment is via the standard treatment for hypothyroidism with thyroxine replacement. Oral T sub 4 sub , or in especially acute cases liothyronine sodium liothyronine , a sodium salt of T sub 3 sub . Hormone replacement in these patients usually reverses the psychotic symptoms, but may not help with cognition cognitive deficits caused by changes in metabolism metabolic activity in the Central nervous system CNS . The myxedema part of the name simply refers to the non pitting edema common to hypothyroidism. Hashimoto s Encephalopathy is a very rare condition associated with Hashimoto s thyroiditis . It was first described in 1966. It is classified as a neuroendocrine disorder. Up to and including 2005 there have been almost 200 case reports of this disease. Up to 1990 only 8 cases reports including the original case had been published. Between 1990 and 2000 43 cases were published. More cases are being reported now, this suggests that this rare condition is likely to have been undiagnosed in the past. MRI s in combination with CSF test are utilized for the diagnosis. Lancet 2 512 514 ref The patient was a 48 year old man with hypothyroidism , multiple episodes of encephalopathy, stroke like symptoms and Hashimoto s thyroiditis confirmed by elevated anti thyroid antibodies. Definition A relapsing encephalopathy occurring in association with Hashimoto s thyroiditis, with high titers of anti thyroid antibodies. Clinically, the condition may present one or more symptoms. Onset is gradual and may go unnoticed by the patient and close associates to the patients. Symptoms can resolve themselves within days to weeks, leaving a patient undiagnosed. Epidemiology The prevalence has been estimated to be 2.1 100,000 with a male female ratio of 1 4. The mean age of onset is 44 with 20 of cases presenting before the age of 18 years.
