Image FIPHisto1.jpg frame right FIP infected kidney showing inflammatory response Feline infectious peritonitis FIP is a fatal incurable disease that affects cat s. It is believed by some to be caused by Feline Infectious Peritonitis Virus FIPV , which is a mutation of Feline Enteric Coronavirus FECV Feline Coronavirus FeCoV . Although there appears to be a connection between FIP and feline coronavirus, no clear cause and effect has yet been proven. Experts do not always agree on the specifics of FIP. However, the most common theory is that the normally benign FECV mutates into FIPV. The mutated virus has the ability to invade and grow in certain white blood cell s, namely macrophages. The immune system s response causes an intense inflammation inflammatory reaction in the containing tissues. This disease is generally fatal http www.vet.cornell.edu fhc brochures fip.html . However, its incidence rate is roughly 1 in 5000 for households with one or two cats. A nasally administered vaccine for FIP is available, but controversial and isn t proven to be highly effective. http www.vetinfo.com cencyclopedia cefip.html . Great strides are being made with an experimental polyprenyl immunostimulant being manufactered by Sass and Sass and tested by Dr. Al Legendre. In one case study, a female cat diagnosed with dry FIP has survived 26 months from the date of definitive diagnosis Fall 2009 . A Richter s hernia occurs when the antemesenteric wall of the intestine protrudes through a defect in the abdominal wall. If such a herniation becomes necrotic and is subsequently reduced during hernia repair, perforation and peritonitis may result. Hinchey Classification is used to describe perforations of the Colon anatomy colon due to diverticulitis . Diverticulosis the presence of bowel diverticula is an essentially ubiquitous phenomenon. With age, all people develop out pouching of the bowel wall as pressure from the inside of the bowel pushes the mucosa outwards. The pouches occur where there is a gap between the muscle fibres of the bowel wall. Inflammation of these diverticula singular diverticulum is termed diverticulitis and occcus when the diveritcula become infected. This classically causes pain, bleeding and diarrhoea. There are several complications that can arise from this inflammation and one of the more serious complications of this is perforation of the bowel. If the perforation is very small, it may be contained often referred to by surgeons as a localised perforation . However, if it is not contained it leads to faecal contamination of the peritoneal cavity faecal peritonitis which is often fatal. The Hinchey classification proposed by Hinchey et al . in 1978 ref Hinchey, E.J., Schaal, P.G. and Richard, G.K. Treatment of perforated diverticular disease of the colon. Advances in Surgery 12 85 109, 1978 ref classifies a colonic perforation due to diverticular disease. The classification is I IV Hinchey I localised abscess para colonic Hinchey II pelvic abscess Hinchey III purulent peritonitis the presence of pus in the abdominal cavity Hinchey IV faeculent peritonitis. The Hinchey classification is useful as it guides surgeons as to how conservative they can be in emergency surgery. Recent studies have shown with anything up to a Hinchey III, a laparoscopic wash out is a safe procedure ref Toorenvliet, B.R., Swank, H., Schoones, J.W., Hamming, J.F. and Bemelman, W.A. Leukocyte esterase LE is a urine test for the presence of white blood cell s and other abnormalities associated with infection . White blood cells in the urine usually indicate a urinary tract infection . The leukocyte esterase LE test detects esterase , an enzyme released by white blood cells. Positive test results are clinically significant. The LE test is also used to screen for gonorrhea and for amniotic fluid infections. The combination of the LE test with the urinary nitrite test provides an excellent screen for establishing the presence of a urinary tract infection UTI .
