  1. Endovascular surgery

Refimprove date February 2009 Endovascular surgery is a form of minimally invasive procedure minimally invasive surgery that was designed to access many regions of the body via major blood vessels . Endovascular techniques were originally designed for diagnostic purposes. Basic techniques involve the introduction of a catheter percutaneous percutaneously into a large blood vessel. Typically the blood vessel chosen is the femoral artery or a vein found near the groin. Access to the femoral artery for example, is required for coronary , carotid , and Cerebrum cerebral Angiography angiographic procedures. The catheter is injected with a radio opaque dye that can be seen on live X ray or fluoroscopy . As the dye courses through the blood vessels, characteristic images are seen by experienced viewers and can assist in the diagnosis of diseases such as atherosclerosis , vascular trauma, or aneurysms . In recent years, however, the development of intravascular balloons, stents and coils have allowed for new therapies as alternatives to traditional surgeries such as Coronary artery bypass surgery CABG , carotid endarterectomy and aneurysm clipping. 1. Endovascular coiling

Endovascular coiling , or endovascular emobilization , is a surgical treatment for cerebral aneurysm s. It was developed in 1991 by Guido Guglielmi at Ronald Reagan UCLA Medical Center UCLA . It is one of two surgical treatments for aneurysms, the other being surgical clipping . Procedure The treatment uses detachable coils made of platinum that are inserted into the aneurysm using a microcatheter. A variety of coils are available. In the case of wide necked aneurysms, a stent may be used as well. The treatment works by promoting blood clotting around the coils, eventually sealing the aneurysm and reducing pressure on its outer wall. There are a number of coil types used in the procedure. These include Guglielmi Detachable Coils GDC which are platinum, Matrix coils which are coated with a biopolymer, and hydrogel coated coils. Studies A number of studies have questioned the efficacy of endovascular coiling over the more traditional surgical clipping. Most concerns involve the chance of later bleeds or other recanalization. ref J Neurointerv Surg. 2011 Apr 27. Orphan date October 2008 The International Symposium on Endovascular Therapy ISET http www.ISET.org is an educational meeting for physicians, scientists, engineers and allied health professionals working in the multidisciplinary field of peripheral and cardiac interventional medicine. The meeting provides information on the latest techniques and technology for the diagnosis, treatment and prevention of vascular diseases, among them stroke , heart attack , aneurysm and hardening of the arteries. The meeting highlights life saving procedures that are considered less invasive than traditional surgery. The five day course was founded in 1989 by internationally acclaimed interventionist Barry Katzen, M.D., a member of the team that in 2005 used stents to repair the leg aneurysms of Dick Cheney Vice President Dick Cheney . Dr. Katzen is known worldwide for his work using angioplasty and stent s to clear blockages in arteries throughout the body. He was one of the first to popularize the use of clot busting drugs for vascular disease. Dr. Katzen is also the founder and medical director of the Miami based Baptist Cardiac & Vascular Institute BCVI http www.baptistheart.net , which presents the meeting. BCVI embraces and ISET promotes the idea that physicians of differing specialties interventional radiologists, interventional cardiologists and vascular surgeons must work together in support of high quality patient care. The ISET meeting is presented annually in South Florida and draws attendees and prominent faculty speakers from around the world. ISET 2011 was held at the Fontainebleau Miami Beach from January 16 20. The 2012 meeting will also be held in that location, from January 15 19. Endovascular aneurysm repair EVAR was compared with open surgical repair. Comparison included cost, mortality, complications, durability, and health related quality of life. At the end of 1 year, there were no differences in health related quality of life. At the 4 year follow up, the two groups had similar rates of all cause mortality. EVAR group had a persistent significant but moderate reduction in aneurysm related deaths. In the EVAR group 41 experienced postoperative complications compared with 9 in open repair group . 1. Sinus pericranii

Sinus pericranii SP is a rare disorder characterized by a congenital or occasionally, acquired epicranial venous malformation of the scalp. ref name Bolognia cite book author Rapini, Ronald P. Bolognia, Jean L. Jorizzo, Joseph L. title Dermatology 2 Volume Set publisher Mosby location St. Louis year 2007 pages 886 isbn 1 4160 2999 0 oclc doi accessdate ref Sinus pericranii is an abnormal communication between the intracranial and extracranial venous drainage pathways. Treatment of this condition has mainly been recommended for aesthetic reasons and prevention of hemorrhage. Definition Sinus pericranii is a venous anomaly where a communication between the intracranial dural sinuses and dilated epicranial venous structures exists. That venous anomaly is a collection of nonmuscular venous blood vessels adhering tightly to the outer surface of the skull and directly communicating with intracranial venous sinuses through diploic veins. The venous collections receive blood from and drain into the intracranial venous sinuses. The varicosities are intimately associated with the periostium, are distensible, and vary in size when changes in intracranial pressure occur. Etiology The nature of this malformation remains unclear. Congenital, spontaneous, and acquired origins are accepted. The hypothesis of a spontaneous origin in the current case of SP is supported by no evidence of associated anomalies, such as cerebral aneurysmal venous malformations, systemic angiomas, venous angioma dural malformation, internal cerebral vein aneurysm, and cavernous hemangiomas. Treatment The surgical treatment involves the resection of the extracranial venous package and ligation of the emissary communicating vein. In some cases of SP, surgical excision is performed for cosmetic reasons. The endovascular technique has been described by transvenous approach combined with direct puncture and the recently endovascular embolization with Onyx. ref Vasc Endovascular Surg. 2011 Jan 45 1 103 5 ref Se ...   more details

  1. Cirsoid aneurysm

Multiple issues unreferenced January 2010 orphan January 2010 A cirsoid aneurysm is the dilation of a group of blood vessel s due to congenital malformation s with AV arterio venous shunting. Cirsoid means resembling a varix . Sometimes, a minor traumatic episode, such as a fall or bump on the head, can lead to the formation of a cirsoid aneurysm. Oftentimes these are trivial traumatic episodes. It most commonly occurs over the head . It can also occur in places where medium vessels lie over bones without much intervening tissues between them and the skin. The superficial temporal artery is the most commonly involved artery. Cirsoid aneurysms of the scalp were first described in 1833 by Brecht. 4 Only 10 20 of these arteriovenous fistula s develop following penetrating trauma to the scalp. In 90 of patients, the superficial temporal artery is the main supply to the fistula with only one dominant feeding artery in 71 of patients. 3 In the remaining cases, there is usually involvement of both the superficial temporal and Occipital artery occipital arteries . 3,4 Untreated patients can develop progressive scalp and facial cosmetic deformity from the markedly tortuous subcutaneous vessels. However, this condition is not life threatening. Surgical resection of the fistula is usually successful, as was for this patient. Endovascular and percutaneous occlusion of the fistulas have been described. However, the results have been mixed. The problem with an arterial approach is that there is recruitment of surrounding normal arteries following occlusion of the arterial feeder and draining venous structures. Arterial approaches may not often be successful in occluding the entire fistula . The results of endovascular occlusion are dependent on the angioarchitecture of the fistula, the supplying arteries and draining venous structures. Arterial approaches may not often be successful in occluding the entire fistula due to the problem of multiple feeding arteries being recruited to ...   more details
