The railway lines through Beresfield are straight and level, and visibility of the signals would have been good. The flashing tail light of the stationary coal train may have been hard to see, as such signals are typically less bright than stationary railway signals. The coal train that went through the red signal had a crew of two, who unusually had been rostered on the same shift for a long period of time. This practice is not desirable since it reduces safety through diversity, wherein one crew member feels alert when the other feels drowsy, and vice versa.     Unreferenced date November 2006 In the Kindu rail accident on November 29, 2005, at least 60 people were killed when they were swept off the roof of a train into the river below as the train crossed a bridge in the eastern Democratic Republic of Congo . After speaking with local officials in his region, Koloso Sumaili, the governor of Maniema province, says it seemed there were over 60 dead in this accident. He said the accident happened around 200  km south of the town of Kindu , as the train travelled towards the southern town of Lubumbashi Trains that travel on that line have lots of people and goods on the roof. When the train was crossing over a bridge, the beams supporting the bridge swept people and goods off the train and into the river below, he said. A British Rail Class 506 Class 506 electric multiple unit train from Manchester Piccadilly to Hadfield, Derbyshire Hadfield had started away from a signal when a railroad switch set of points moved underneath the train, causing it to be derailed and throwing the centre carriage onto its side. Two passengers were killed and 13 were injured. Investigation The investigation found that the signalman rail signalman at Stockport Junction signal box was at fault. The lever frame at Stockport Junction was equipped with electro pneumatic slides instead of levers, with electrical economising contacts located above them. The signalman wanted to cross a freight train travelling in the opposite direction behind the passenger train, but the points were locked by a track circuit until the passenger train was well clear of them. In order to defeat the interlocking and speed things up, he had pulled the points slide slightly out of the frame. This action held the protecting signal at red as a safety measure, so in order to clear it, he obtained a false feed between two of the economiser contacts, most likely using a Bardic lamp or a broken hinge that was subsequently found. The one fatality was Albert Workman the driver of the mineral train a married man with two children. Also injured was Ernest Day, aged 24, of Cardiff, fireman on the mineral engine.
